Everyone resident in Japan from 40 pays into this, including foreign residents, so your parent is almost certainly already enrolled. Paying premiums does not open services; certification does, and that is a separate municipal step you apply for. Families abroad often assume coverage is automatic on diagnosis. It is not. The card in the drawer is the starting point, not the key.
